Location: Lunenburg, Mecklenburg, Brunswick, and parts of Charlotte and Halifax counties

Position Overview

We are seeking a dedicated and adaptable Case Manager/Community Support Services Specialist to support individuals with acquired brain injuries and their families through a combination of community-based case management and direct support services. This role blends traditional case management responsibilities with hands-on support in daily living and community integration. The Case Manager/Community Support Services Specialist works under the direction of the Programs Manager, with support from the Director of Programs, and plays a key role in helping clients build independence, develop functional skills, and successfully engage in their communities. This position requires both independent work and frequent interaction with clients in home and community settings. Comprehensive training is provided to ensure your confidence and success in the role.

What You’ll Do

Intake & Assessment:
  •  Conduct comprehensive intake evaluations and gather required documentation 
  •  Assess client strengths, needs, supports, and available resources 
  •  Present cases to the Intake Review Board and determine service appropriateness 
  •  Identify assistive technology needs and appropriate supports 
Client & Direct Support Services:
  •  Build and maintain therapeutic relationships with clients and families 
  •  Provide one-on-one support in home and community settings 
  •  Assist clients with functional skill development, including daily living and social skills 
  •  Support clients in developing and working toward individualized goals 
  •  Maintain regular contact to monitor progress and provide guidance 
  •  Advocate for clients across systems and services 
Service Coordination & Monitoring:
  •  Coordinate and monitor services aligned with client goals 
  •  Facilitate support teams, including referrals and service planning 
  •  Connect clients with long-term supports as needed 
  •  Evaluate progress and determine when services are no longer required 
Compliance & Documentation:
  •  Maintain accurate and timely documentation in accordance with agency standards 
  •  Ensure compliance with confidentiality and regulatory requirements 
  •  Manage client-related resources within approved budgets 
  •  Support efforts related to CARF accreditation 
Outreach & Community Engagement:
  •  Participate in outreach, education, and public awareness activities 
  •  Build and maintain relationships with community partners 
  •  Support referral development and service awareness efforts 

What We’re Looking For

Skills & Experience:
  •  Knowledge of brain injury and its impact on individuals and families 
  •  Understanding of case management principles and independent living strategies 
  •  Ability to support clients with activities of daily living and community integration 
  •  Strong communication, organization, and time management skills 
  •  Ability to work independently while managing a dynamic caseload 
  •  Proficiency in basic computer systems and documentation tools 
Education & Requirements:
  • Bachelor’s degree in a related field (e.g., rehabilitation, psychology, human services) 
  • At least 1 year of case management or direct service experience (brain injury experience preferred) 
  • CBIS, QBISP, or similar certification (or ability to obtain within required timeframe) 
  • Valid driver’s license and reliable personal vehicle 
  • Current CPR/First Aid certification (or ability to obtain at our expense) 
  • Background free of disqualifying offenses related to abuse or neglect
  • Must reside within the service area of Lunenburg, Mecklenburg, or Brunswick Counties.
  • Must be available every month (typically every third Thursday) for an in-person staff meeting in Wytheville from 10am-2pm. This meeting is considered work time with mileage paid.
  • Must be available for in-person onboarding the first week of hire at our main office in Roanoke. Hotel accommodations provided.
Work Environment & Physical Requirements
  •  Primarily home office-based with frequent travel throughout the service area 
  •  Regular in-person services provided in client homes and community settings 
  •  Frequent driving required (mileage reimbursed) 
  •  Ability to communicate effectively across in-person and virtual settings 
  •  Ability to remain stationary or move as needed to perform job duties 
  •  Occasional lifting up to 50 pounds, with or without assistance
