Now Hiring: Full-Time Community-Based Case Manager
Location: Franklin County, Patrick County, Virginia

Are you passionate about making a real difference in the lives of others? Brain Injury Solutions is seeking a dedicated, full-time Case Manager to support survivors of acquired brain injury in their journey toward recovery and independence. This community-based position involves working directly with individuals and families to identify personal goals and connect them with resources that empower long-term success. The Case Manager will participate in ongoing marketing and outreach activities to promote agency programs, strengthen partnerships with community providers, and generate appropriate client referrals. This is a home office position, but face-to-face visits with clients in the aforementioned location is required. Comprehensive training is provided to ensure your confidence and success in the role.

Starting Salary: $43,000/year with the opportunity to increase after 90 days based on performance.

What We’re Looking For:
Required Qualifications:
  • Bachelor’s degree in social work, counseling, human development, therapeutic recreation, or a related field.
  • Strong communication skills.
  • Proficiency with Microsoft Office 365.
  • Valid driver’s license and reliable transportation for day travel within the service area.
  • CPR/First-Aid certification (or the willingness to obtain certification at our expense).
  • Must reside within the service area of Franklin or Patrick Counties.
  • Must be available every month (typically every third Thursday) for an in-person staff meeting in Wytheville from 10am-2pm. This meeting is considered work time with mileage paid.
  • Must be available for in-person onboarding the first week of hire at our main office in Roanoke.

Preferred Qualifications (not required to apply):
  • Experience with brain injury survivors or in a related field.
  • Community-based case management experience.
  • Knowledge of rehabilitation principles or comparable experience.
